9 2021 Community Report Flex Farm Upgrades - Lutheran Food Pantry: $2,928 To upgrade four Flex Farms units in order to increase yields and reduce volunteer/staff time. Recipient: Lutheran Food Pantry Flex Farm - Waushara County Food Pantry: $4,445 To purchase one Flex Farm unit from Fork Farms to ensure a consistent supply of fresh produce in this underserved area. Recipient: Waushara County Food Pantry Samaritan Counseling Capacity Building: $20,000 To improve operational efficiency and client services by strengthening technology infrastructure for electronic medical records, insurance billing, donor and grant management, and staff training. Recipient: Samaritan Counseling Center of the Fox Valley, Inc. Flex Farm - Boys & Girls Club of Oshkosh: $4,445 To purchase one Flex Farm unit from Fork Farms to teach children how to grow fresh produce, which will supplement the Club's meal program. Recipient: Boys & Girls Club of Oshkosh, Inc. Flex Farm - Winneconne Community School District: $4,445 To purchase one Flex Farm unit from Fork Farms to supplement the existing two units in order to ensure a consistent supply of fresh greens to students and staff while providing educational opportunities and engagement around growing fresh food. Recipient: Winneconne Community School District Fall Prevention: $77,610 (Year 1 of 3, $150,000 total) To provide no-cost home safety modifications and repairs for low-income older adults to help prevent falls. Collaborators: Rebuilding Together Fox Valley, Oshkosh Fire Department, Neenah- Menasha Fire Rescue, Regional Trauma Advisory Council, Finding Balance Together, Aging & Disability Resource Center Waushara SHINES Capacity Building: $50,287 (Year 1 of 2, $117,304 total) To bring the Waushara SHINES program coordinator from a part-time to full-time position to improve efforts to measure and document engagement, offer more activities and events for the recovery community, and conduct outreach and education in the community. Collaborators: Unity Recovery Services, Waushara SHINES Home Visitation for High Risk Families: $26,081 (Year 1 of 3, $79,560 total) To hire a part-time Family Support Specialist in order to serve additional Oshkosh area families through the home visitation program with a focus on enrolling single parents and parents with mental health concerns. Recipient: Family Services of Northeast Wisconsin Apricity Capacity Building: $125,000 To support staff additions in order to complete Medicaid pre- screens, assessments and authorizations; improve staff cultural competency given substantial increases in the number of Native American clients; and pilot a family sober living house. Recipient: Apricity Reach Counseling COVID Relief: $30,000 To provide operating support to help reduce counseling service wait times, continue to build culturally responsive services, and offset decreases in Federal Victims of Crime Act (VOCA) funding. Recipient: Reach Counseling Services Elementary Sources of Strength Program: $3,000 To purchase the new elementary school Sources of Strength curriculum and support the training of two staff advisors to lead the program. Recipient: Tri-County Area Schools Traveling Closet: $50,000 To pilot a program to bring new clothing, bedding, and hygiene items to Oshkosh Area School District students identified by school social workers via monthly distributions at the Boys and Girls Club of Oshkosh. Collaborators: Community Clothes Closet, Boys & Girls Club of Oshkosh, Oshkosh Area School District Christine Ann COVID-19 Relief: $25,000 To continue to offer emergency shelter, 24/7 hotline services, legal advocacy, and outreach to those who are struggling with the effects of domestic abuse by providing operating support to offset decreases in Federal Victims of Crime Act (VOCA) funding and private donations. Recipient: Christine Ann Domestic Abuse Services, Inc. Waushara County Food Pantry Capacity Building: $75,000 To hire a 0.5 FTE executive director and 0.5 FTE program coordinator and move towards a sustainable succession plan for this rural pantry that also serves as a food hub for 12 other rural pantries. Recipient: Waushara County Food Pantry Oshkosh COTS Transitional Housing: $115,000 (Year 1 of 3, $325,000 total) To provide individuals experiencing homelessness with a safe and affordable place to live, food, household needs, health care and training on basic living skills, enabling them to transition to self-sufficiency and independent living by replicating the (Appleton) COTS program in Oshkosh to better serve individuals in Winnebago county. Recipient: COTS, Inc. Employee Benefits - Day by Day Warming Shelter: $36,661 To start offering employee benefits in recognition of the COVID-related health risks to employees and the need to offer competitive wages and benefits for staff recruitment and retention. Recipient: Day by Day Warming Shelter MULTI-YEAR GRANTS Transition to Success for Justice-Involved Individuals: $96,326 (Year 2 of 3, $246,830 total) Link justice-involved individuals with a case manager to help them successfully move from a correctional facility back into the community and navigate their career and educational journeys to sustainable wage jobs. Collaborators: Forward Service Corporation and Wisconsin Department of Corrections Probation and Parole School District of Omro's Mental Wellness Program: $78,380 (Year 2 of 3, $247,307 total) Launch Omro NOW (Nurturing Omro's Wellness), a system that provides screening, education and the necessary tools to help all students and staff feel mentally, emotionally and socially prepared for life's ups and downs. This multi-pronged approach will increase mental health programming and access to services and create a culture of mental health awareness while reducing stigma. Collaborators: School District of Omro, Samaritan Counseling, N.E.W. Mental Health Connection, NAMI Oshkosh, Collaborative Wellness Head Start Early Childhood Mental Health: $106,436 (Year 2 of 3, $301,000 total) Improve Head Start students' social-emotional and self- regulation skills by deepening teachers' use of Conscious Discipline techniques and embedding coaching and mental health supports in the classroom and via home visits with families. Conscious Discipline is a research-based classroom management system that addresses teachers' social- emotional skill sets and internal states first and then empowers them to teach and model these skills with students. Collaborators: UW Oshkosh Head Start, Catalpa Health Teens 2 Work: $80,100 (Year 2 of 3, $237,073 total) To expand Teens 2 Work into a year-round program that will provide teens with safe, supportive work experiences at partnering employers. Employment barriers, like transportation, that teens might normally face in a rural community will be addressed and eliminated through this program. Collaborators: Boys & Girls Club of the Tri-County Area, Green Lake Conference Center, Drexel Building Supply Center, and Crossroads Market Early Childhood Development Screening: $80,900 (Year 2 of 3, $256,200 total) Early Expansion: To improve outcomes for young children living in Winnebago County by increasing the number of children that receive developmental screenings and increasing the number of families that are connected to resources and care through a Centralized Access Point. Collaborators: First Five Fox Valley, Child Care Resource & Referral, Early Intervention Program, Winnebago County Health Department, Oshkosh Area School District, ThedaCare, Davis Child Care Center, Family Services of Northeast Wisconsin, and UW Oshkosh Head Start Mentor 2.0: $85,000 (Year 3 of 4, $400,000 total) Implement a nationally successful, technology- enhanced, vocational mentoring program within Oshkosh North High School that provides individualized support and guidance to empower students to succeed in and after high school. Collaborators: Big Brothers Big Sisters of the Fox Valley Region and Oshkosh Area School District Mentoring Program Expansion: $117,700 (Year 3 of 3, $286,500 total) Grow the mentoring program that was piloted in the Berlin Area School District in 2018-19 for youth ages 9-17 and launch a similar program partnership with the Green Lake Area School District. Collaborators: Boys & Girls Club of the Tri-County Area, Berlin Area School District, and Green Lake Area School District Healthy Families Waushara County: $44,510 (Year 3 of 3, $94,938 total) Offer voluntary nurse home visiting services to parents facing challenges such as single parenthood, low income, and current or previous issues related to substance abuse, mental health issues, and/or domestic violence. Collaborators: Waushara County Health and Human Services Departments, CAP Services, Aurora Health Care, ThedaCare Rock the Block Oshkosh: $20,000 (Year 4 of 4, $300,000 total) Empower residents to revive their low income neighborhoods and enhance their quality of life by working together on home repair, preservation and beautification projects. Collaborators: Habitat for Humanity of Oshkosh, Greater Oshkosh Healthy Neighborhoods, and City of Oshkosh Winnebago Catch-A-Ride: $38,000 (Year 2 of 3, $118,000 total) To hire a workforce development facilitator to reach out and connect with employers, drivers and riders in order to provide more employment transportation services to individuals who do not qualify or have access to existing transit programs in Winnebago. Collaborators: GO-EDC Foundation, Forward Service Corp., ThedaCare, Lutheran Social Services, Feonix Mobility Rising Early Childhood Programming Alliance: $60,045 (Year 2 of 3, $175,000 total) To deliver home visiting services for pregnant women and families with children up to age one that are experiencing risk factors and facilitate community-based parenting programs in Green Lake County. Collaborators: Children's Hospital of Wisconsin Community Services, Green Lake County Department of Health & Human Services, Forward Service Corporation
