11 CAP Services Fresh Start Enhanced: $63,062 (Year 2 of 3, $188,888 total) Expand the Fresh Start Program to include training related to renewable energy to increase the potential for program graduates to secure life-sustaining jobs. Collaborators: CAP Services, City of Waupaca, Northwind Renewable Energy, Waupaca Foundry, Faulk Brothers Construction English Language Learning Staff: $15,000 (Year 2 of 3, $45,000 total) To expand organizational capacity by adding an English Language Learning staff member to improve the literacy skills of low income adult learners who are new in our country. Recipient: Fox Valley Literacy Council Appleton Ready to Read: $15,000 (Year 3 of 3, $45,000 total) To improve kindergarten readiness of Hmong and Hispanic children in Appleton, an indicator of long-term academic success. Collaborators: Friends of Appleton Public Library, Appleton Area School District Kids Succeed: $30,000 (Year 2 of 3, $68,500 total) To help children living in neighborhoods of concentrated poverty realize academic and personal success, and self-sufficiency. Collaborators: Sustainable Fox Valley, Columbus Elementary School ServiceWorks Program Expansion: $18,362 (Year 3 of 3, $123,398 total) To expand the ServiceWorks program, enrolling ex-prisoners and helping them develop work skills and references while working to create public park space and gardens at Riverview Gardens. Collaborators: Riverview Gardens, Ryan Community Sex Trafficking: $43,500 (Year 2 of 3, $100,500 total) To support development and early implementation of a strategic plan for dealing with sex trafficking of juveniles in Outagamie County. Collaborators: Sexual Assault Crisis Center, Appleton and Grand Chute Police Departments, Harbor House, Outagamie County District Attorney, etc. PATH Program Continuation: $50,000 (Year 2 of 2, $100,000 total) To continue the PATH Program, which provides in-school mental health services, until it reaches the point of sustainability. Collaborators: United Way, Catalpa Health, Family Services of Northeast Wisconsin and Lutheran Social Services of Wisconsin and Upper Michigan Thrive Career Academy: $50,000 (Year 2 of 3, $150,000 total) To develop the Thrive Academy to give individuals with disabilities the skills and support necessary to obtain and keep employment, thereby becoming self-sufficient. Collaborators: Valley Packaging Industries, Community Care Positive Youth Involvement: $15,000 (Year 3 of 3, $45,000 total) To help Youth Go serve the increased number of youth coming to the facility for guidance and support. Recipient: Youth Go Sober Living: $25,000 (Year 2 of 3, $150,000 total) To open a sober living house providing peer recovery support, helping 18-22 people a year, struggling with addiction, leave treatment and build new lives. Collaborators: Step Industries, The Mooring Programs, Nova Counseling Services Day Treatment: Filling a Gap for Fox Valley Youth: $80,000 (Year 2 of 3, $300,000 total) To improve pediatric mental health care by launching a day treatment center that provides intensive on-site mental health services for youth in grades 7 to 12. Collaborators: Catalpa Health, CESA 6 Oral Health for Children: $15,000 (Year 3 of 3, $45,000 total) To introduce a new third grade screening and orthodontics program. Recipient: Tri-County Community Dental Clinic Job Training and Placement for Veterans: $57,122 (Year 2 of 3, $177,803 total) To provide a structured job training program and job placement for veterans who have been offered an alternative to incarceration by the court system. Collaborators: Riverview Gardens, Outagamie County Veterans Treatment Court Comprehensive School-Based Mental Health: $61,000 (Year 2 of 3, $300,000 total) To implement E3 (Engage, Educate, Empower), a comprehensive school-based mental health program led by a group of mental health providers in Hortonville Area School District. Collaborators: Catalpa Health, Samaritan Counseling Center, NAMI Fox Valley and Family Services of Northeast Wisconsin Housing Partnership of the Fox Cities: $10,000 To host a community event where the Housing Coalition's homelessness and near homelessness research is released. Collaborators: Housing Partnership of the Fox Cities, Warming Shelter, LEAVEN, etc. Housing Partnership of the Fox Cities: $34,000 To ensure broad-scale action and community impact from Project RUSH (Research to Understand and Solve Homelessness) by hiring a project manager who will implement and oversee the work in eight strategic areas of focus. Collaborators: Homeless Connections, Fox Valley Warming Shelter, Members of the Housing Coalition Greater Fox Cities Area Habitat for Humanity: $93,121 (Year 1 of 3, $300,000 total) To develop a new neighborhood revitalization program with "Rock the Block" events. Home repairs, preservation projects, improved relationships between neighbors and increased neighborhood pride will result in a significant shift from a culture of poverty. Collaborators: Rebuilding Together – Fox Valley, Neighborhood Partners, City of Appleton Rebuilding Together – Fox Valley: $76,100 (Year 1 of 3, $291,462 total) To implement a collaborative intake process for home repair/renovation services with Habitat for Humanity. The program will increase the number of low-income homeowners and seniors served, keeping them from losing their homes and allowing them to use their resources to provide for their own basic needs. Collaborators: Rebuilding Together – Fox Valley, Greater Fox Cities Area Habitat for Humanity LEAVEN: $63,211 (Year 1 of 3, $174,726 total) To use outcomes of a Basic Needs Giving Partnership planning grant to implement a new no-fee loan program with a financial counseling requirement. Collaborators: LEAVEN, FISC Riverview Gardens: $116,740 (Year 1 of 3, $272,142 total) To expand ServiceWorks and reach out to individuals in crisis through a new job training program that will take place in downtown Appleton.
