Recipient: Women's Fund for the Fox Valley Region Youth Go: $58,500 Advance and enhance the work of Youth Go. Recipient: Youth Go Bridge the Gap, Inc.: $20,000 (Year 1 of 3, $100,000) Bridge the Gap's (BTG) mission is to provide a safe, non-judgmental environment for those with autism and all special abilities to reach their full potential as the journey from childhood through adolescence and into adulthood. The Bridge the Gap Academy will meet the social, emotional, physical, mental and trauma needs of each specific student by providing evidence based learning options that are tailored to each specific student. BTG will provide options to area school districts to close deficit gaps in serving students in special education as well as provide ongoing advocacy to parents, families and individuals across the lifespan. Recipient: Bridge the Gap Diverse and Resilient: $73,005 (Year 1 of 3, $296,415) To add a Community Education and Outreach Coordinator to support the growing needs in the LGBTQ community in the Fox Valley & region and the organizations serving them. Recipient: Diverse and Resilient Foundations For Living: $139,000 (Year 1 of 5, $417,417) To fund an Assistant Director, Program Manager and Office Manager so Foundations for Living has the necessary capacity to more effectively serve and empower people in the Waupaca area who are in financial crisis, addiction recovery, or homelessness through advocacy, education, emotional support, and resources. Recipient: Foundations for Living Rock the Block - New London: $75,000 Repair and improvement projects that address health, safety, and energy efficiency of homes for low- income and elderly homeowners. Recipient: Greater Fox Cities Area Habitat for Humanity Harbor House Domestic Abuse: $65,000 (Year 1 of 3, $195,000) To expand Harbor House's legal advocacy program in an effort to shrink the divide of justice and to better serve clients who are already facing financial abuse, racial disparities, social inequities, and no access to legal representation. Recipient: Harbor House Domestic Abuse Holyland Food Pantry: $250,000 To build a barrier free food pantry, which would permit the pantry to better address root causes, enhance access to emergency and supplemental food, resource referrals, private consultation and educational opportunities for the service area known as the Holyland. Recipient: Holyland Food Pantry NeighborWorks Green Bay: $43,500 (Year 1 of 3, $113,000) Support for Neighborhood Partners' merger with NeighborWorks Green Bay over a three year transition period. NeighborWorks Green Bay will fully incorporate Neighborhood Partners' asset-based community development approach to building neighborhood social connections and social capital across its service territory. Funding will support expenses related to work in the Fox Valley region. Collaborators: NeighborWorks Green Bay, Neighborhood Partners NEW Hmong Professionals: $100,000 (Year 1 of 3, $300,000) To start and manage several new programs and continue and sustain existing programs to meet the basic needs of the Hmong community in the Fox Cities area. These programs span needs in health & wellness, family and social connectedness, and education. Recipient: NEW Hmong Professionals Rebuilding Together Fox Valley: Falls Prevention Program: $77,610 (Year 1 of 3: $150,000 total) To provide no-cost safety modifications and repairs for low-income older adults to help prevent falls. Recipient: Rebuilding Together Fox Valley St. Vincent de Paul of Appleton: $87,934 (Year 1 of 3, $263,800 total) To restructure the current Spanish-speaking Resource Advocate position into two separate positions with the addition of a bilingual coordinator; ensuring real system changes occur while providing a sense of belonging and hope in our community. Recipient: St. Vincent de Paul of Appleton The Samaritan Counseling Center of the Fox Valley, Inc.: $231,544 To support THRIVE Calumet, a collaboration to provide on-site mental health services in Brillion, Chilton, Hilbert, New Holstein, and Stockbridge school districts in partnership with Samaritan Counseling Center of the Fox Valley, Calumet Medical Center, and the Calumet County Behavioral Health Unit. Recipient: The Samaritan Counseling Center of the Fox Valley Unity Recovery Services, Inc: $90,671 (Year 1 of 3, $209,239 total) To support organizational/ administrative development and building capacity of Unity Recovery Services' Fox Valley PRISM Team program. The majority of this funding request is intended to bring two individuals that are currently in part-time positions to full-time status.
